- Biodata page & latest Malaysian entry stamp (if any)
- Police clearance certificate (translated + certified by Malaysian embassy)
- Resume (employment history, company name, experience)
- Proof of income (3-month salary slips + employer letter + bank statements)
- OR Proof of passive income/business/investment (annual tax return, latest 12 months statements)
- Medical report (In Malaysia)
- Valid health insurance coverage in Malaysia
- Certified marriage/birth/divorce/adoption/legal custody documents (translated if necessary)
- Letter from doctor if dependent is disabled (21+ years)
